Now that you have reviewed the qualifications necessary to receive a grant from K21 and determined that your organization is eligible, the next step is to complete the application. You can download or printout our application below. Please carefully read the instructions on the application and proceed accordingly. Please Note: The required second signature, in addition to the applicants', depends upon the organization type, but should be an authoritative overseer of the organization and financial resources. Examples are:

• Non- Profit Organizations:  Officer from your Board of Directors
• Governmental Organizations:  Elected official with financial decision-making responsibility such as City Mayor, County Councilman, Township Trustee, or Town Board Member
• Public School Entity:  School Superintendent
